Your safety is of utmost importance to us. For that reason we need you to please read through this Medical Questionnaire before you attend one of our scuba dive courses. We would like you to do this in order to establish whether you are healthy enough scuba dive with us.

The purpose of this Medical Questionnaire is to find out if you should be examined by your doctor before participating in recreational diver training. You don’t have to sign the form now. You are only required to read through it to establish if you will need to consult a doctor. We however require that you sign the form prior to your tuition beginning (you can do this at our Scuba Dive Centre in Cape Town).

 A positive response to a question does not necessarily disqualify you from diving. A positive response only means that there may be a pre-existing condition that may affect your safety while diving and that you must seek the advice of your physician prior to engaging in scuba dive activities.

So, if your answer to every question is negative, then we can start with your scuba dive training. If you should answer positive to a questions, please first consult your doctor, before attending a scuba dive course.
